Meet Carol VanHorn
My many years of pharmacy experience include corporate and independent community pharmacy practice sites, hospital care, and long term care consulting, as well as business management in various areas.
I am certified in Diabetes Care Management, Cardiovascular and Hypertension Management, Asthma and other Respiratory conditions, Immunizations, Delivering Medication Therapy Management Services, and Pain Management and SUD.
I am a graduate of Oregon State University College of Pharmacy and completed a residency in Geriatric Pharmacy Practice from University of Washington along with an MBA. I have served as Affiliate Faculty for OSU, UW and Pacific University Schools of Pharmacy. As an advocate for healthy Oregonians, I have volunteered with student pharmacists and other community organizations at several immunization clinics and health outreach events.
Meet Irene Croswell
I am certified in Delivering Medication Therapy Management Services, Diabetes Care Management, Immunization, Travel Health Services, Point of Care Testing, Pharmacogenomics, Integrative Health / Alternative Medicine, and Tobacco Cessation. I am also trained in lung health management for people living with asthma or COPD.
My original professional degree was a Bachelor of Science in Pharmacy. Being a believer in lifelong learning, I returned to school and earned my PharmD from University of Colorado.
I have always enjoyed getting to know the people I am privileged to care for, working with their healthcare teams, teaching student pharmacists, and connecting /volunteering with the communities where I have lived and worked.
